Payment Responsibilities

Co-Pays: Your insurance company
requires you to pay your co-payment at the time of service. If no co-payment is received
at the time of service an additional administrative charge of $25 will be billed.
Self-Pays: If no
payment is received at the time of service an additional administrative charge of $25 will
be billed.
General:
You are financially responsible for all co-pays, coinsurance,
deductibles and/or services that are denied or not covered by your insurance carrier.
Attention
Medicare Patients
Medicare has changed
its rules for payment of your lab tests and will not pay for some tests that your doctor
considers important. Medicare will not pay for tests:
When they are part
of a routine physical or are screening for early detection of disease
If you have had the
same test too often
If the test is not
medically necessary based upon your doctors diagnosis; or if the test is considered
to be for research or experimental use only.
